Details of Activity
Briefly Describe what is intended.  For clarity, this may include sketches/story board/diagrams/checklists
Recreation of a scene from TV Series Fargo. It will be filmed in and around a small elevator. Storyboards and checklists will be used in order to help with the filming process.




Hazards Identified and Risks Arising
Identify and list what could reasonably cause harm.  Against each identify who is at risk, i.e. who could be harmed and how. Ignore the trivial, concentrate only on those hazards that could result in serious harm or affect several people.

·       Actors and crew in a small confined space at once.
·       Equipment on the floor- cause hazard to passers-by
·       Lift will still be in operation so need to be able to exit when required
·       Moving around of equipment. - requires lifting of more than 2 heavy items.
·       Ensure all food/drinks are kept away from any electrical equipment.
·       Failure to notify members of the public entering the lift that there will be a fake gun, could cause people to be shocked and take unnecessary action which can be avoided.

Risk Assessment and Proposed Precautions
For each of the above, evaluate the risks and decide whether existing precautions are adequate or more needs to be done.  Take into account information from contractors, premises management, resource providers, and others about the risks and controls.  List the proposed controls for each significant hazard and identify any contingency plans in place for emergencies or failures of safety critical arrangements, e.g. miss fire of explosive effect or stunt; car going off road; member of public being door-stepped, turning violent.  Include fire and first aid and welfare arrangements.

All equipment will need to be stored in a safe place when not in use. When in use, no loose wires must pose a threat of injury (ensure all wires are kept safe and passers by are aware they are there)

As lift is still in operation, ensure doors aren’t open for a long period of time to prevent alarms going off.  

Ensure equipment is looked after and there are not broken pieces left on the floor.

Ensure safety when climbing to reach crane shot- use another person for support

Fake gun will be used. Ensure all use of gun is kept inside of elevator and away from the public at all times.
Ensure public is notified before entering elevator of the fake gun.

Producer - the term “Producer” refers to the person in control of the production, and not only to those with a “Producer” job title or grade.  On any programme the Producer is responsible for safety.  This responsibility covers all staff, contributors, contractors or others who may be affected by the production activity.  The Producer can delegate duties to others, e.g. EM, gaffer, 1st AD, but responsibilities cannot be delegated. The producer is responsible for ensuring that the person to whom he/she delegates a duty has sufficient experience, knowledge and status to perform it safely.
